Record requests

Requests for medical records must be made in writing and are subject to a processing fee of 75 cents per page for page 1-25, and 50 cents per page for pages 25-100. Immunization records and all medical records can only be released after a signed medical information release form has been submitted to our office.

Forms

Requests for form completion for day care, school, camp, sports participation, etc. are best handled at the time of the visit. However, most forms can usually be completed if the child has had a physical within the past 12 months. We ask that you allow 48 hours for forms to be completed, if not completed at the time of your visit.
