• Back-to-school physicals
As the summertime begins to wind down, the school year winds up! Are your child’s immunizations up to date? Be sure that your child is prepared. Call today to schedule your child’s back-to-school physical!

• Need a PHYSICAL FORM or IMMUNIZATION RECORD?
Requests for physical forms and/or immunization records require a 24 hour turnaround. We can fax or mail any forms after we have received a signed Authorization of Medical Release from you. (We will not be able to release any medical forms without one!)
We are unable to complete physical forms based on physicals performed by other medical offices, including urgent cares.
Be sure to find out if your child has had a physical within the last year. Most insurance carriers will only cover ONE routine physical per year.
As we get closer to the 25th of August, our schedule will become booked up! Schedule your child’s appointment today!

Welcome to Starks Pediatrics at Mallard Creek
Dr. Starks founded Starks Pediatrics at Mallard Creek in 2007 with the goal of bringing children's healthcare back to the community, back to each family knowing and trusting their doctor.
Dr. Starks sought to create a healing village for children which provides for all of a child’s health care needs from well child care to outpatient management of acute and chronic illness. The philosophy of our healing village is that each child and family deserves respectful, experienced, and trusted care. Dr. Starks has worked tirelessly to provide just that type of care to children and has brought together a group of compassionate and experienced staff to assist in creating an environment in which children feel welcomed and nurtured.
